Map controls missing + incorrect view of the buble

First of all, thank you for this great extension!

I encountered an issue:
I installed the extension, but I noticed that there is an incorrect view of some elements. Could you please help me on this?

I cannot find the solution. Maybe it's due to the template or to something else.

I reinstalled the extension many times, but didn't work.

I use joomla 2.5
