How do I put the contact form in my website?

I installed the extension. I can see it in the "Components" menu. I can access there all kind of options. But they only reffere to the google maps settings.
Now... I want to display a contact form and the google map in my CONTACT US page in joomla.
How do I do that? I cannot find any info on this.
